for some circumistances related to hospital appointment = because I have a hospital appointment
at the same day of your class = correct English would be "at the same time as your class / on the same day as your class" but you don't actually need these extra seven words because your sentence starts with "I cannot come to your class tomorrow".
Always assume that your reader is intelligent!
